Background

The prescriptions were right; the follow-through was not. Patients with chronic conditions were missing doses and running out of medication, and their outcomes slipped as a result.

Approach

We set up WhatsApp medication and refill reminders tuned to each patient's plan, with simple nudges to take doses and reorder before running out. The reminders were bilingual and light-touch, designed to help rather than nag, and stayed inside a channel patients already used every day.

Results

Patients stayed on their medication more consistently, refills happened before gaps opened up, and clinicians saw fewer avoidable setbacks between visits. For chronic care, that steadiness is where real outcomes live.
